RN Case Managers
Forum Health at Home is seeking full time Case Managers to provide skilled nursing care. One position is available in for Trumbull County and another for Mahoning County.
These positions will be responsible for the case management and coordination of care for all assigned cases. These are field staff positions.
Current Ohio RN license. Requires minimum of 1 year registered nursing experience. Home Care experience preferred.
