You’ll start working with your care team right away.

First, you’ll spend an hour with a Forum Health physician.

Next, you’ll have a one-hour consultation with a health coach. They’ll analyze lifestyle factors — including nutrition, exercise, and sleep — to help develop your treatment plan.

If you decide to move forward with membership, payment for your welcome visit will also cover these follow-up items:

  • A basic set of lab tests received by all Forum Health members
  • A 20-minute virtual session with a health coach

We might order additional labs based on your physician assessment.

Our cash-based membership plans aim to provide freedom and certainty. Insurance billing and reimbursement for labs and services not included with membership might vary by plan, patient, and other factors. Please contact us if you have any questions.

A Forum Health plan does not replace insurance coverage. While insurance is not required to meet with a doctor or select a plan, it often covers a portion of the cost for lab work and other services.

Forum Health membership is not intended to replace your relationship with a primary care physician. For some referrals and services — like an annual physical, mammogram, or colonoscopy — they might be better positioned to meet insurance requirements. We recommend that you find or remain established with a primary care physician.

Some! New patient welcome visits include a basic set of laboratory tests. Members will continue to receive these basic labs annually at no additional cost.

Additional lab tests are not included in your membership fee, but they might be covered under your health insurance plan.
